Note, we are not currently accepting or waitlisting new clients for medical (Specialist GP) care. 

Our Dietitian, Romy, is accepting new clients for in clinic and telehealth consultations. Please click here to book in for a consult or a free 10-minute discovery call.

We are now accepting waitlist requests for Osteopathy services from clients wishing to access interdisciplinary team care at CHA. Please click here to request a place on the waitlist

Existing

Existing Client Bookings:

If you are an existing client wanting to rebook a consult with a clinician who you have seen before, click here.

This portal is not for new clients. If you book a consult with a CHA Clinician who you are not under the current care of, your consult will be removed. Note that if you are seeking to re-engage in care with a CHA clinician who you have previously seen, you will be considered a new client.

 Dr Thomas is currently on clinical leave. We will notify exisitng clients when bookings become available again

If you are an existing client seeking to add new CHA Clinician/s to your treating team, complete â€‹the 'Existing Client File Transfer & Triage Form' here to indicate this. Note that some clinicians may have a waitlist in place.

 

If you have previously completed this form and you:

  • did select the Clinicians you would like to add to your team in the relevant section, you do not need to complete any additional. paperwork as we will have received your request.

  • did not select the Clinicians you would like to add to your team, email us at admin@connectedhealthalliance.org.au to amend this.

This form is for existing current clients of CHA Clinicians only (not for new clients, or clients who have been previously discharged). 

Rescheduling a Session.

Consults can be cancelled online. The link for this can be found on the consult confirmation email sent automatically by our system Cliniko when a consult is booked.
