CHA Patient Journey.

At Connected Health Alliance Wollongong we offer a truly interdisciplinary approach to Hypermobility healthcare.

 

We believe the best outcomes for our patients come from collaboration, which is why we bring together multiple clinicians to work as a cohesive team.

 

Every Friday, our team meets for dedicated case conferences to discuss your care, ensuring that all perspectives are considered and that you benefit from a practical, well-rounded treatment plan.

 

This collaborative model not only leads to better results for our patients but also supports our team in preventing clinician burnout – ensuring that the care you receive is consistently comprehensive, meaningful and patient-centred.

Who can book in with Connected Health Alliance?

At this stage we can only support clients who are able to attend in-person for at least an initial consult and every six months thereafter. Note, some therapies are offered in person only, but Telehealth sessions are available by negotiation with your Clinician depending on your individual management plan, capacity and goals.

Our waitlisting and triage process.

NOTE: Our New Client Waitlist is not yet open, and we currently do not have an estimated launch date. Thank you for your patience and understanding.

New Client Waitlist Form process

​

When the New Client Waitlist is released, we will ask you complete this to allow us to triage your care, and place you on the waitlist with CHA Clinicians that you are requesting. We will contact you as consults become available with your selected Clinicians. Where possible, we will aim to provide you with a rough wait time estimate, however as many of our Clinicians live with invisible illnesses, these may become delayed at any time for a variety of reasons.

Current wait time estimates for new clients (updated 14/04/25)

  • Specialist GP - wait time TBD, as our GP is currently prioritising existing CHA clients and previously waitlisted clients. 

  • Physiotherapy - currently at capacity, wait time TBD. If you have not seen our Physiotherapist in 6+ months, please wait to join the new client waitlist.​

  • Dietetics - available with minimal wait.

  • Lymphatic Therapy - available with minimal wait. 

  • Osteopathy - 1-3 months, service stating early March, available on Fridays.

We will reach out to you with the New Client Waitlist Form, 1-2 weeks before releasing this to the general public. In the meantime, you are welcome to book if with Clinicians who have immediate availability and a self-booking option.

A note on our waitlisting process

We aim to maintain a fair system for processing waitlisted clients. Please do not email us your medical history, symptoms or other details before we request these, as this will not expedite your waitlist position.
